In November, a 70-year-old woman died after riding Revenge of the Mummy, an indoor roller coaster at Universal Studios in Orlando, Florida.

News of the unidentified woman’s death was detailed in the Florida Department of Agriculture and Consumer Services’ quarterly report on theme park injuries, the Orlando Sentinel reported on Jan. 16.

The woman reportedly became unresponsive while riding an indoor roller coaster on Nov. 25 and later died at a nearby hospital. Florida’s report “reflects only information reported at the time of the incident,” and Florida’s government “has not received an update on the initial assessment of the occupant’s condition.”

Universal Orlando media representatives did not immediately respond to USA TODAY’s questions about the incident.

NBC affiliate station WFLA in Florida reported that 21 incidents have been linked to the Mummy’s Revenge Ride since it opened in 2004, including reports of nausea, dizziness and spinal fractures.

Universal Studios Florida’s website describes Revenge of the Mummy, located in the New York area of the theme park, as a “high-speed plunge into pitch darkness.”
