The cause of Ozzy Osbourne’s death has been revealed.

According to his death certificate obtained by the New York Times and the Sun, Black Sabbath Locker died at the age of 76 from a heart attack.

The death certificate reportedly listed Osborne’s cause of death as “from hospital cardiac arrest,” “acute myocardial infarction,” “coronary artery disease, and Parkinson’s disease with autonomic dysfunction.” Myocardial infarction is a medical term for heart attacks.

USA Today has contacted representatives of Osbourne and General Register Office.

Rockstar passed away on July 22nd, just weeks after he performed a farewell show with Black Sabbath. His family previously confirmed in a statement to USA Today.

“We are saddened as well as telling us that we must report that our beloved Ozzy passed away this morning,” the family statement said. “He was with his family and surrounded by love. We ask everyone to respect the privacy of our family at this time.”

Osborne was battling Parkinson’s disease in the last year of his life after being diagnosed with degenerative syndrome in 2019. According to the Mayonnaise Clinic, symptoms of Parkinson’s disease include tremor, slowing movement, stiff muscles, loss of balance, balance and auto-movement.

“It’s not a death sentence for stretching your imagination, but it affects certain nerves in your body,” explained Osborne’s wife, Sharon, when she appeared together with him in 2020’s “Good Morning America.”

Osborne also underwent surgery to the neck after being injured in 2019 when he collapsed at his Los Angeles home. He postponed all tour dates with his Tour No More Tours II concert tour, which opened at GMA after the fall, when his “GMA” was recovering from a “severely challenging” year.

“I had to do surgery on my neck so I screwed all the nerves,” he said. “I’ve had numbness in this arm since surgery. My legs are getting cold. I don’t know if it’s Parkinson’s or what it’s… that’s a strange feeling.”

On SiriusXM in February, Osborne shared that he can no longer walk. But he said at the time, “I might be moaning about how I can’t walk… But when I looked down the road, there were people who didn’t do half of my thing and they didn’t make it.”

Despite his health challenges, Osborne was able to perform while sitting on the throne during Black Sabbath’s “Back to the Star” fairwell concert held in Birmingham, England on July 5th. The performance was filmed for a concert film and documentary scheduled for release in 2026.

“Thank you from the bottom of my heart,” Osborne said on the show. “You’re the best, each of you.”
